NTSB Hearing Reveals Instrument Errors, Communication Failures in Deadly DCA Collision

On July 30, 2025, the National Transportation Safety Board (NTSB) began a three-day investigative hearing into the tragic midair collision near Ronald Reagan Washington National Airport (DCA) on January 29, 2025.

The crash, which claimed 67 lives, involved PSA Airlines flight AA5342, an American Airlines regional jet, and a U.S. Army Black Hawk helicopter.

The hearing’s first day revealed critical insights into potential causes. It highlighted instrument errors, communication failures, and systemic issues in the congested DCA airspace.

Instrument Errors and Pilot Oversight


Experts at the hearing pointed to a combination of instrument errors and pilot oversight as key factors. The helicopter’s altimeters, which measure altitude, may have malfunctioned, displaying conflicting data.

A radio altitude reading showed the crash occurred at 278 feet, despite regulations limiting the helicopter to 200 feet.

This discrepancy suggests that faulty equipment or misinterpretation by the crew could have played a role.

Additionally, the helicopter crew was likely wearing night-vision goggles, which may have limited their ability to scan their surroundings effectively, further contributing to the collision.

Systemic Failures in Focus


The hearing exposed troubling systemic issues involving the U.S. Army and the Federal Aviation Administration (FAA).

Air traffic controllers at DCA had proposed relocating the helicopter route where the crash occurred, citing safety concerns. However, management dismissed the suggestion, raising questions about oversight and risk management.

NTSB Chairwoman Jennifer Homendy has long warned about the dangers of DCA’s crowded airspace. This busy airspace sees commercial, military, and government aircraft operate in close proximity.

She noted that “close calls” between helicopters and planes occurred monthly from 2011 to 2024. Homendy described the situation as an “intolerable risk.”

Communication Breakdowns


A critical communication failure also emerged as a factor. Investigators revealed that at least one radio transmission from DCA air traffic control was likely “stepped on”. In other words, it was interrupted or not received by the helicopter crew.

This missed instruction could have prevented the crew from adjusting their position, contributing to the collision.

The hearing is examining the training and guidance provided to controllers to understand how such breakdowns occur in high-pressure environments.

Evidence and Ongoing Investigation


To provide clarity, the NTSB presented compelling evidence, including an animation of the crash, radio communications, and video footage of the final moments.

These materials help illustrate the sequence of events leading to the tragedy. The hearing’s agenda over the next two days (July 31 and August 1, 2025) includes analyzing the helicopter’s data systems, the complexities of DCA’s airspace, and controller training protocols.

The NTSB emphasizes that the hearing is a fact-finding mission, not an effort to assign blame. A final report, expected in 12 to 18 months, will determine the probable cause and recommend safety improvements to the FAA.
